Training Description

This interactive training introduces participants to the Social Determinants of Health (SDOH) framework as a foundational model for stabilizing system-involved and foster youth. Utilizing trauma-informed principles, case-study analysis, and collaborative care-mapping techniques, the course explores the intersections of environmental adversity, housing instability, and health disparities. Attendees will discover how to operationalize structural screening tools to effectively mitigate the impacts of toxic stressors. Participants will gain practical, systemic navigation skills directly relevant to improving placement stability, resource access, and long-term wellness outcomes for system-involved youth.

Learning Objectives

• Explain two evidence-based, trauma-informed screening practices designed to safely identify adverse environmental factors without causing re-traumatization for system-involved youth.

• Integrate a minimum of four closed-loop community referral pathways into an individualized, multi-system stabilization care plan for a high-risk foster youth.
