The Enhanced Care Management (ECM) Program, operating under the umbrella of Fred Finch CARES, is a comprehensive care coordination benefit launched in July 2023 under CalAIM (California Advancing and Innovating Medi-Cal). ECM adopts a Whole Person Care Approach to healthcare, providing physical, behavioral, emotional, and social services to enhance care coordination and health outcomes. It is a care-coordination service offered to children, youth, and their families insured through Managed Medi-Cal. With a cross-regional services delivery model in both Northern and Southern California, ECM aligns and coordinates services across multiple systems of health and behavioral healthcare while addressing social determinants.
Families with children, teens, or young adults up to 21 years old, youth up to 26 years old if involved in the Foster Care System, children, teens, or young adults enrolled in CCS/CCS Whole Child Model, children, and families at risk of/or experiencing homelessness, and children with mental health or substance use conditions are eligible to enroll in the ECM program.
The program offers a thorough assessment incorporating the Social Determinants of Health, tailored services to meet each family's unique needs, dedicated care manager for each enrolled family, and care coordination, outreach and engagement, health promotion, comprehensive transitional care, linking families to services, advocacy, and access to resources.
